A proper incident investigation process is just what any safety system needs in a workplace.
The law states that all employers need to do everything reasonably practicable to remove the risks and hazards in the workplace. They simply cannot get away from this rule.
Should accidents and incidents appear, a proper incident investigation should be done in order to establish what transpired, who was at fault, or what procedures caused the event to take place.
Once that has been completed, the information must then used to update the safe operating procedure in question, so that the incident will not happen again.
Let us say that for every 20 near-misses safety incidents on the same process, there will be a possible 1 major incident, so we conduct incident investigations to prevent the one major incident from taking place.
Let's put it another way, let's say an employee was injured badly or worst, died, the court of law will ask you if you did everything to remove that hazard or risk. You can state that you have safe operating procedures in place, however, the government might ask to see your incident record to see if something like this has happened before, even if it was just a small incident, you then will be asked why did you not fix your work process when the hazard or risk first appeared.
In other words, let's say Paul was bumped by a forklift in the warehouse, nothing came of it other than bumps and bruises, however no incident investigation was done. A month later, another employee in the same warehouse was driven over by the forklift. You will have to stand in court, and explain why you did not do something about it when Paul was knocked.
Ok, maybe it won't be that bad, you could possibly try to hide the fact that Paul was knocked, but remember, when it is a serious offence, the department of labour lawyers and inspectors will come knocking and interviewing employees or watch security footage, then as they say, the truth always comes out, and you would not want to be on the wrong end of the stick in Health and Safety at the workplace.
Did you know, the penalties for non-compliance is a massive fine, or a minimum of 2 years in jail, or both.
So let's rather protect you and your employees, and get someone trained up to investigate incidents, that way both you and your employees can have more confidence in the workplace, as they will no doubt see and notice the safety efforts put in by the investigator and change of safe operating procedures.
This is a process that is followed after an accident or incident has occurred in the workplace. The objectives of an incident investigation is to establish the event that took place and the causes. That information is used to improve safe operating procedures in the workplace.
